STEPS TO ENROLLMENT

Thank you for your interest in Scottsdale Academy Private School of the Arts. The steps to enrollment for the 2010-2011 school year are as follows:

1. Schedule a tour and classroom visitation.

2. Submit the following:

	Non-refundable $100.00 Application Fee
	Application
	Blue immunization/information form
	Picture/emergency form (in handbook)
	Class program form (in handbook)
	Enrollment contract
	Current immunization records (including Hepatitis A)
	Ballet form (for Monet and Picasso students)

Tuition is subject to a $25 late fee if not received on the due date. Please make
checks out to Scottsdale Academy. Tuition is due for days a child is not in attendance
including illness, vacations and holidays. Scottsdale Academy is open Monday
through Friday 7:00am-6:00pm. There is a late fee of $5 per 5 minutes for parents
arriving after 6:00pm. This late fee is due at time of pickup. Please respect the
needs of our staff to get home with their families. Extra days are charged at $50.00 per day.

5 days ~ $715.00 per month
4 days ~ $675.00 per month
3 days ~ $635.00 per month

Mini Mozart (Ages 2)
Marvelous Monet (Ages 3)
Picasso Pals (Ages 4-5)

5 days ~ $665.00 per month
4 days ~ $625.00 per month
3 days ~ $585.00 per month

Elementary (Kindergarten-4th Grade) Summer Camp Only

Extended Day for all classes is from 3:00 pm to 6:00 pm and is charged at
$5.00 per hour of use.

After care can be used for days not in attendance at your convenience.

Tuition includes:

Before School Program (7:00am - 9:00am)
All scheduled enrichment classes: Ballet, Chorus, Creative Dance, Spanish Immersion, Piano, Art, P.E., Sign Language, Pilates, Gardening, Gymnastics, etc.
All beverages.
